HAIR TEMPLE SERVICE WAIVER & RELEASE OF LIABILITY

I understand that hair treatments, colouring, and styling services may involve the use of chemicals, tools, and equipment that carry certain risks, including but not limited to allergic reactions, scalp irritation, hair breakage, colour variation, or damage resulting from pre-existing conditions or previous treatments.

HEALTH & CLIENT RESPONSIBILITY

  • I confirm that I have disclosed all relevant medical conditions, allergies, and sensitivities that may affect my service.
  • I understand that failure to disclose relevant information (e.g., use of medication, henna, chemical relaxers, or scalp conditions) may increase risk of adverse reactions.
  • I acknowledge that hair condition, texture, and previous treatments may affect the outcome of any service, and that results may vary.
  • I understand that achieving or maintaining specific results may require ongoing treatments and at-home care products as advised by the stylist.

CHEMICAL & COLOUR SERVICES

I understand that chemical and colour treatments may alter the structure of my hair. While Demi Ellis and Hair Temple stylists take all reasonable precautions, I acknowledge that results cannot be guaranteed and that damage, dryness, or breakage may occur due to factors beyond the stylist’s control (such as prior chemical services or at-home treatments).

ASSUMPTION OF RISK & RELEASE

In consideration of receiving services from Demi Ellis Hair Temple, I voluntarily assume all risks associated with salon treatments, including but not limited to cuts, colour, extensions, or product use. To the fullest extent permitted by law, I release and hold harmless Demi Ellis, her assistants, and the venue from any claims, demands, damages, or liabilities arising from or related to these services, whether caused by negligence or otherwise.

MEDIA CONSENT

I understand that photographs or videos may occasionally be taken for educational or promotional use (e.g., social media or website). I consent to the use of my image and likeness unless I notify the stylist in advance by selecting the opt-out option on this form or at the appointment.

BEHAVIOUR & SAFETY

I agree to follow all reasonable salon policies, health and safety instructions, and COVID-19 or hygiene protocols. I understand that failure to comply with these guidelines may result in the refusal of service or future bookings being declined.

DATA & RECORDS

I consent to the collection and secure storage of this waiver and my contact information for record-keeping, insurance, and health & safety purposes. Records may be retained for a reasonable period (e.g., up to 6 years) and handled in accordance with applicable data protection laws. MINORS

If the client is under 18, a parent or legal guardian must provide details and sign this waiver prior to any service being performed.

ACKNOWLEDGEMENT
